Palliative Care Social Worker

PURPOSE OF THIS POSITION The Palliative Care Social Worker is the team member responsible for providing consultation to the interdisciplinary team and direct service to home health or hospice patient/families to assist in identifying, understanding, and meeting their psychosocial needs.
The Social Worker understands and is committed to the philosophy, goals, and objectives of the Hospice concept and can explain these to families requiring assistance in decision making and understanding care options.
The Social Worker assists patients with access to community resources for home health or hospice related needs and manages on-call as scheduled in a consistently responsible manner.
JOB DUTIES/RESPONSIBILITIES Duty 1:
Demonstrates knowledge and skills necessary to provide age specific care to the patients ser ved with Bridge Home Health, Hospice and Palliative Care Duty 2:
Maintains assigned patient caseload within scheduled hours while minimizing overtime.
Manages on-call responsibilities to include keeping management informed of any unusual or special incident occurrences.
Duty 3:
Makes initial contact with each assigned palliative care patient/family to assess psychosocial needs and to set mutual goals.
Duty 4:
Establishes and maintains communication/collaboration with the Interdisciplinary Team and patient's primary physician regarding patient mental or psychosocial condition, plan of care, and anticipated needs to ensure exceptional patient care.
Duty 5:
Able to communicate palliative care Services and discuss payment options.
Duty 6:
Utilizes a team approach to teaching opportunities and family care issues of the patient.
Duty 7:
Utilizes educational materials or outside resources as needed.
Cooperates with other agencies in coordinating patient care with other agencies and community resources.
Duty 8:
Facilitates interventions for the patient/family making ethical and legal decisions to manage patient care.
Duty 9:
Documentation is consistent with or exceeds Bridge Home Health and Hospice guidelines.
Duty 10:
Assists patients/families, colleagues, and self to cope with grief and loss.
Able to disengage in a professional manner.
Duty 11:
Displays Service Excellence as evidenced by practicing the mission, vision, and values of the organization to promote patient satisfaction.
Duty 12:
Complies with organizational policies, procedures, and practices to ensure quality job performance and Medicare/Medicaid compliance.
REQUIRED QUALIFICATIONS Master's degree in Social Work Licensed Social Worker (LSW or LISW) by the State of Ohio Good interpersonal skills, knowledge of family dynamics and counseling skills are essential.
Knowledge of and commitment to the philosophy, mission and goals of the agency and the standards and regulations governing it.
A valid drivers license is required (if you do not have a valid Ohio driver's license you must obtain one within 30 days of your residency in the state).
You must also meet BVHSs company fleet policy and insurance company requirements, and any other requirements that may be required to operate a vehicle.
Positive service-oriented interpersonal and communication skills required.
Individual must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served on his/her assigned unit/department.
The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient status.
Must be able to interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and to provide the care needed as described in the area's policies and procedures PREFERRED QUALIFICATIONS Previous experience with terminally ill persons is required, hospice experience preferred.
Previous counseling experience preferred.
PHYSICAL DEMANDS This position requires a full range of body motions with intermittent walking, lifting, bending, squatting, kneeling, twisting, and standing.
The associate will be required to walk for up to one hour a day, sit for four to six hours a day and stand for one hour a day.
The individual must be able to lift twenty to fifty pounds and reach work above the shoulders.
The individual must have good eye-hand coordination and fine finger dexterity for simple grasping tasks.
The individual must have excellent verbal communication skills to perform daily tasks.
The associate must have corrected vision and hearing in the normal range.
The individual must be able to operate a motor vehicle for business travel and community involvement.
This position is classified at risk for possible occupational exposure to blood borne pathogens (HBV, HIV, etc.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
